As a full service Home Health Care Agency, we pride ourselves in being an industry leader offering a broad range of services and programs to assist our elderly and disabled clients in the comfort of their own homes. We coordinate with the medical community, family members, and caregivers to provide superior quality service in the most caring manner. Our qualified and experienced staff includes skilled nurses, certified case managers, licensed social workers, home health aides, therapists, and dieticians.

Our medical and non-medical home care services include:

Case Management Services
Case managers conduct a comprehensive assessment of client needs and, in partnership with the client, develop a plan of care, provide guidance, and monitor the provision of care and services.

Nursing Care Services
Specializes in performing skilled nursing evaluations, giving physical assessments, developing plans of care, monitoring client conditions/status, and coordinating care/services with physicians.  Professional nurses also perform skilled procedures, such as wound care, as needed; provide instruction to clients and family members in disease processes and management; provide medication management; and patient education with regards to diet/nutrition and safety.
                                  
Physical Therapy Services
Specializes in improving functional limitations, which involves establishing a home exercise program designed to improve mobility, strength, range of motion, and pain management. Physical therapists may also educate caregivers with the goal of expediting healing and preventing re-injury.

Occupational Therapy Services
Specializes in treatment to assist in regaining lost motion and in restoring the ability to perform activities of daily living, such as dressing, grooming, bathing, and eating, after an accident or illness. This therapy will involve establishing a home exercise program designed to improve strength and range of motion.

Speech Therapy Services
Specializes in the treatment of swallowing, speech, and language disorders. It may include physical exercises to strengthen the muscles used in speech (oral-motor work), speech drills to improve clarity, or sound production practice to improve articulation.

Clinical Social Work Services include:

Gerontological Counseling
Provides emotional support, information, and guidance, including mutual support groups for older adults who are having mental, emotional, or social adjustment problems arising as a result of the process of aging. Through counseling, clients are helped to maximize the use of available resources to assure physical and emotional well-being.

Mental Health Counseling
Focuses on the treatment of psychiatric disorders and rehabilitation for impairments of persons suffering from a mental illness, including depression and anxiety.

Medical Social Workers
May provide information and/or referrals to community resources for assistance and resolve social or emotional problems which may interfere with the treatment of medical conditions or recovery. 

These services may also include long term planning, assistance with advanced directives, caregiver counseling, conflict resolution, and grief and/or end of life issues.

Nutritional Assessments/Nutrition Risk Reduction Services
Provides guidance, education, and care to clients and caregivers in the preparation and eating of appropriate, nutritionally balanced meals and promoting better health. These individualized services are designed for individuals who are experiencing weight loss, have trouble swallowing, or are diabetic.

Home Health Aide Services
Provides personal care and assistance with activities of daily living such as: bathing, dressing, care for hair and teeth, assisting with exercises, ambulation, transfers, and repositioning. Other duties may include changing bed linens, light cleaning, and meal preparation during visits. Home Health Aides are supervised by Registered Nurses and Physical Therapists.

Personal Care Services
Provides assistance with bathing, dressing, and personal hygiene.

Homemaker Services
Includes housekeeping, laundry, meal planning and preparation, and routine house-hold activities by a trained homemaker, only when the caregiver and client are physically unable to provide the service.

Escort Services
Provides personal accompaniment (not transportation) to or from service providers, medical appointments, or other destinations needed by the client.

Companionship Services
Non-medical supervision and one-to-one socialization designed for clients who are lonely and in need of human contact.

Respite Services
Non-medical assistance and supervision provided to a client for the purpose of giving relief and rest to a family member/caregiver.

Chore Services
Includes household chores, such as washing floors, windows, walls, and appliances in order to maintain a clean, sanitary, and safe living environment.

Home Delivered Meals
Provides appropriate and nutritionally balanced hot, cold, or frozen meals served in the home with flexible options including: low fat/low sugar, diabetic, low sodium, pureed, and ethnic preferences, such as Kosher, American, and Spanish.

Consumable Medical Supply Services
Includes the provision and delivery of usable supplies such as adult disposable diapers, personal wipes, lotions, bandages, nutritional supplements, and other consumable supplies.

Medical Equipment
Includes the provision of assistive devices, such as special recliners, hospital beds, walkers, wheelchairs, canes, and adaptive telephone devices.

Environmental Modification Services
Physical adaptations to the home necessary to ensure the health, welfare, and safety of the client. Adaptations may include the installations of ramps and grab-bars, widening of doorways or bathroom facilities, and converting tubs to showers.
                            
Emergency Alert Response Service
Includes the installation and service of an electronic device that enables clients to secure help in an emergency, 24 hours, 7 days a week. The system is connected to the home phone and programmed to signal a response center once the “Help” button is activated. Clients may also wear a portable device to allow for mobility.

TeleHealth Monitoring Services
Provides a hospital grade electronic device to allow individuals to take vital signs readings, as needed, including temperature, pulse,  blood pressure, weight, oxygen levels, and/or blood sugar levels. This information is transmitted from the device to our agency, where a Registered Nurse reviews the data, identifies appropriate action, and communicates with the client’s physician and family, as needed.

Basic Subsidy
A fixed sum cash payment made to an eligible caregiver each month to reimburse some of their expenses of caring for the client. A basic subsidy is provided for support and maintenance of the care recipient, including medical costs not covered by Medicaid, Medicare, and any other insurance.

Adult Day Care Services 
Provides an organized day care program of therapeutic social and health activities and services assisting client in restoring or maintaining optimal care. This service gives caregivers the opportunity to receive a rest.

Services such as Dental, Hearing, Vision, and Assisted Living Facilities can be provided to those eligible to participate in our Long Term Care Diversion Program.

We administer a broad range of public and privately funded assistance programs and accept Medicare, Medicaid, private pay, and long term care insurance.
