November 11, 2008 United HomeCare Services Leads National Expansion of Medication Management Improvement System for Homebound Elderly

United HomeCare Services will be the first of four national demonstration sites selected by California-based Partners in Care Foundation to replicate its evidence-based Medication Management Improvement System (MMIS) thanks to a grant from the Health Foundation of South Florida. Initially piloted by Partners in Care Foundation in California, MMIS has proven effective in reducing medication errors that jeopardize the health of frail community-dwelling elders by capturing four of the most common medication errors:

1. Unnecessary therapeutic duplication
2. Use of medications that increase risk of uncharacteristic confusion
3. Use of medications that increase dizziness and risk of falls
4. Inappropriate use of Non-Steroidal Anti-Inflammatory Drugs
