In our mission to promote independence and wellness for frail elderly and disabled adults, United HomeCare® embraces interdisciplinary partnerships with physicians and other medical professionals, hospitals and other healthcare facilities and academic institutions.

Over the years, we have developed partnerships with a number of organizations – some have grown from pilot projects to ongoing programs. From care transitions across settings to disease management, telemedicine and other emerging technologies, we are open to collaboration in a number of areas, with the goal of improving the lives of those we serve.
