Job Description:
The Registered Nurse is responsible for direct patient care in the home under the supervision of the physician and Clinical Director.

DUTIES AND RESPONSIBILITIES:
1. Provide skilled nursing care to patients in their home under the physician’s plan of treatment established for the patient.
2. Plan with the Nursing Supervisor for continuity of care of the patient according to physician's orders and the established patient care plan.
3. Functions within the scope of the Nurse Practice Act.
4. Makes the initial evaluation visit to a patient.
5. Is in contact with physicians for orders or change of orders.
6. Keeps adequate and current records and reports as required on the patient assigned.
7. Supervises and teaches other health care personnel.
8. Keeps up-to-date records, clinical, and progress notes pertaining to the patient’s status and condition.
9. Is knowledgeable regarding agency policies, rules, and regulations regarding Medicare, Medicaid, and other sources.
10. Knowledgeable about available community resources.
11. Initiates appropriate preventive and rehabilitative nursing procedures.
12. Assumes responsibility for personal growth and development through:
a. active participation in nursing organizations; and
b. participating in continuing education programs relating to the position.
13. Performs related duties as assigned.
14. Assist in coordinating treatment for Agency’s clients and/or consumers. (HIPAA)
QUALIFICATIONS AND SKILLS:
o Graduation from an accredited School of Nursing and maintain current license from the State of Florida. Public health nursing certification preferred.
o At least one year of general medical/surgical experience.
o Knowledge of the practice of community health nursing and concept of team nursing.
o Must be able to organize, manage, and function in assigned areas.
o Must have understanding and knowledge of the development of plans for patient care and the ability to participate in them.
o Good verbal and written communication skills.
o Ability to perform tasks involving physical activity, which may include heavy lifting and extensive bending and standing; and ability to deal effectively with stress.
o Must have a valid Florida Driver's License.
o Computer knowledge.
