Job Title: Physical Therapist

Job Description: 1. Continuously develop their skills and knowledge by readings, conferences, departmental training programs, or some combination of the above.

2. Develop and display a clear customer service focus at all times.

3. View their work as part of the larger United Home Care Service system, and strive to improve their performance in concert with, rather than in competition with, their co-workers and other United Home Care Service employees.

4. Be conscious of the resources they use in completing their work and to strive to use these resources as efficiently and effectively as possible.

DUTIES AND RESPONSIBILITIES:

1. Performs evaluation for physical therapy by applying diagnostic and prognostic muscle, nerve joint and functional ability tests as medically prescribed for the patient.

2. Provides direct physical therapy treatment as provided by the patient’s physicians’ record. These treatments may include active and passive exercises, massage, heat, etc. Directs and aids patients in active and passive exercises, muscle re-education and gait and functional training, including transfer activities and prosthetic training.

3. Formulates and establishes short-range and long-range goals, plans and treatments.

4. Teaches appropriate treatment procedures and supportive activities to the patient and family and leaves written instructions with the patient, family and home health team-RN and HHA – so that all efforts are aimed toward rehabilitation.

5. Instructs patient in care and use of wheelchairs, braces, crutches, canes, and prosthetic and orthotic devices.

6. Observes records and reports to the physician the patients’ reaction to treatment and any changes in the patient’s condition monthly, or more frequently as necessary within HIPAA guidelines.

7. Is responsible for maintaining all records required by the agency and Medicare, and presents these records within the time frame as required.

8. Participates in patient progress and discharge review, case record review and team conferences as requested.

9. Supervises PTA and HHA as required.

EDUCATION/EXPERIENCE:

1. Graduation from a school of Physical Therapy approved by the Council on Medical Education and Hospitals of the American Medical Association. Certification by the American Registry of Physical Therapists and/or active membership of the American Physical Therapy Association is preferred. Must be licensed to practice Physical Therapy in the State of Florida and maintain current professional liability insurance.

2. At Least one year supervised experience in a recognized agency or institution in Physical Therapy.

KNOWLEDGE/SKILLS:

1. Must have personal and professional integrity. Willing to accept the philosophy, purpose and ideals of the organization. Must posses the personal qualities and abilities to deal with patients and others. Should have an aptitude for and experience in teaching. Able to assemble and maintain records as requested and/or required by the agency. Should have the ability to exercise independent judgment.
